Telehealth is useful for many non-emergency concerns, but it cannot replace every physical examination, bedside test or urgent intervention.

Severe breathing difficulty, chest pain, signs of stroke, collapse, severe bleeding, a severe allergic reaction or another life-threatening problem needs emergency care.
Some problems need hands-on assessment
A doctor may recommend in-person review when diagnosis depends on examination, vital signs, a procedure, dermoscopy, imaging or immediate pathology. The safest next step may change during the consultation.
