Medical Assessments & Documentation for Corporate and Insurance

Fast, clear, consent-based treating-doctor opinions for employers, insurers and compliance teams.

EUCA MD is an Australia-wide telehealth practice supporting workforce documentation needs across employer and insurer contexts. We provide timely treating-doctor documentation following clinical consultation, with clear clinical reasoning and practical recommendations. Our process is designed for reliable turnaround, consent-based communication and professional standards suitable for corporate and insurance workflows. Our doctors have experience supporting documentation requests from national employers and major insurers across Australia.

Retail telehealth bookings are available via Book Now.

What We Provide

  • Fitness for duties letters (full duties or modified duties) following consultation.
  • Return-to-work plans covering restrictions, hours and review timeframe.
  • Capacity and functional limitation statements based on clinical assessment and available records (no functional testing).
  • Employment medical letters aligned to role requirements (where clinically appropriate).
  • Insurance and income protection forms (treating-doctor sections only).
  • Treating-doctor medical summaries for HR or insurer use (with consent).
  • Clarification letters responding to employer or insurer questions.
  • Investigation summaries once pathology or imaging results are received and reviewed.
  • Sick leave and work restriction documentation (where clinically appropriate).

Common Situations We Support

  • Pregnancy workplace adjustments
  • Return-to-work planning
  • Ongoing capacity clarification
  • Insurance form completion
  • Long-term absence documentation
  • Employer follow-up questions

All documentation is issued in our capacity as the patient’s treating doctors following consultation. We do not provide independent medical examinations (IMEs) or medico-legal reports.

How It Works

  1. Employer or insurer submits a documentation request.
  2. The patient books a telehealth consultation.
  3. Clinical assessment and review of relevant records.
  4. Patient consent obtained for any third-party release.
  5. Documentation issued.

We work collaboratively with patients, employers and insurers, with all documentation finalised according to independent clinical assessment.

Turnaround Times

  • Standard letters are typically issued within 24 business hours following consultation, once required forms and role details have been received.
  • Where investigations are required, documentation is issued within 24 business hours of results being received and reviewed.

Timeframes apply once required information and results are available.

Fees

Documentation fees are determined on a case-by-case basis, depending on:

  • Scope and complexity
  • Volume of record review required
  • Form length
  • Urgency
  • Whether investigations are required

A quotation is confirmed prior to consultation.

EUCA MD can support organisations through:

Important Notes

  • We provide documentation in our capacity as the patient’s treating doctors following consultation.
  • For clarity, we provide treating-doctor documentation rather than independent medico-legal opinions or IME-style reports.
  • Capacity statements are based on clinical assessment and available records.
  • All documentation reflects our independent clinical judgement.
